The health insurance industry is constantly changing, and therefore our participation with various insurance organizations' changes as well. To meet your needs on an individual basis, we request that you notify us with any insurance questions or concerns.
Completing insurance forms is a complimentary service that we provide, but it is your responsibility to make sure the proper coverage exists before starting treatment.
You are responsible for all expenses not covered by insurance.
